The EBM codes for billing are expected to be determined by the Evaluation Committee of Physicians and Health Insurance Companies starting in 2026. However, it is important to note that costs will only be covered under strict medical conditions, which we will explain in detail below.

The specific details of billing are determined using so-called EBM codes. The legal deadline for setting and adjusting these codes has now been extended until April 9, 2026.

At the same time, the existing transitional arrangement has been extended: liposuction for stage III lipedema can continue to be billed via the existing GOPs until June 30, 2026.

Based on the current situation, it can therefore be assumed that the new regulation will be gradually implemented in the course of 2026.

  • Liposuction is not permitted for individuals with a BMI over 35 kg/m². In this case, structured obesity treatment is the first priority, as surgical treatment of lipedema can only be medically assessed after weight stabilization.
  • With a BMI between 32 and 35 kg/m², liposuction is only permitted if the excess weight is primarily caused by fat deposits typical of lipedema.

This is determined using the waist-height ratio (WHtR) – the ratio between waist circumference and height. The age-dependent thresholds are:

  • Up to 40 years of age: WHtR ≤ 0.5
  • 41 to 49 years: increase of 0.01 per year of life
  • From age 50: WHtR ≤ 0.6

Which areas of the body are covered by statutory health insurance?

Health insurance currently only covers liposuction on the legs and—if medically indicated—on the arms. Other areas such as the abdomen, back, buttocks, chest, or face are not covered by statutory health insurance. This can compromise the overall aesthetic result and lead to disproportions.

What happens if I do not meet the BMI limits?

If your BMI is above 35 kg/m², your health insurance will not cover the costs. If your BMI is between 32 and 35 kg/m², you must prove that your excess weight is mainly due to lipedema-typical fat deposits on the extremities – this is done using the waist-height ratio. The focus during the six-month conservative therapy should be on weight stability or slight reduction.

How long does the entire treatment take until the surgery?

The six-month conservative therapy is mandatory. Added to this is the time required for the initial diagnosis, the second diagnosis after six months, and approval by the health insurance company. Overall, you should expect to wait at least 7 to 9 months before the operation can be performed.
